Florida Alternative Tax Forms Vendor Information
The Florida Department of Revenue has established guidelines for a company producing alternative tax forms. The information provided should be used by companies
to successfully produce alternative forms that meet the same specifications as official Department forms.
The Department will make every attempt to keep vendors informed of any changes that may occur annually or during the year that could affect your business.
Alternative forms vendors must submit an Alternative Form Vendors and Payroll Processors Development Application
(Form
GT-320227)
annually, prior to the development and/or testing of alternative forms. Once your application is received and processed, you
will be granted access to the Federation of Tax Administrators' (FTA) State Exchange System, where the Alternative Forms
Requirements are stored.
What's New
Alternative Forms Requirements have moved to the Federation of Tax Administrators' (FTA) State Exchange System. The 2022
Alternative Forms Requirements are now available. Refer to the Alternative Forms Requirements Guide, for changes and
requirements.
The Alternative Forms Requirements ─ Include the necessary information to be able to develop Alternative Tax Forms that are
